Services for Referring Doctors

Holistique offers IV services beyond the Holistique IV Lounge menu. Physicians outside of Holistique may refer their patients for IV's not listed here. Mostly the IV for referring physicians are for chronic conditions and goes beyond wellness. You may list the IV(s) in the form as succinct as possible.
All referring physicians may fill out the form “Referring IV patients”, fax (1-833-371-1483) or email (info@holistique.com). It is always helpful to follow that up with a phone call to confirm and review the order.
If you are a provider and want to refer your patient to Holistique Medical Center for IV therapy, we are happy to be of service to you and your patients. Below is the process:
1. Referring provider completes our referral form
2. Fax completed form to 1-833-371-1483
3. Holistique provider reviews form
4. If approved, referred patient will be contacted and scheduled for IV administration.
